Westinghouse Vectrol Controls

We have a 250 HP, 480VAC Westinghouse Vectrol Soft Start motor starter that powers up with a Phase Loss Fault. Phase to Ground voltages of ~280VAC, are present on all three phases at the line terminals of the starter. In addition the phase to phase voltages are measured at ~485VAC.
